What is a High Lactate?If the lactate is greater than 4, then this identifies septic shock on its own. More importantly the higher the lactate the less likely the patient is to survive.
As we start to resuscitate the patient with fluid, if we are doing well then we will see the lactate begin to fall. If the lactate doesn't drop with fluid resuscitation it is likely that the patient will require invasive monitoring. |
Fluid ChallengesStart fluid challenges early and carry out serial lactates. If fluid challenges are carried out correctly and the lactate isn't improving, then you have a big problem, ITU needs to be informed early.
